GREAT NEWS! Players can now download and print copies of LOF's Waiver/Release forms and save time during registration. These forms are good for (1) year and will be kept on file at LOF. Future visits (within that year) to LOF will only require each player to 'sign in' on a daily player log.
ALL players must have a Waiver/Release form (page 1) and a Model Release form (page 2 - lower half) on file at LOF to play. BOTH forms are MANDATORY to gain access to our fields. These forms may be obtained by clicking below:

All players under 16 must also have a Medical Permission Form (page 2 - upper half) on file at LOF to play. |
