LOF Waiver Forms

GREAT NEWS! Players can now download and print copies of LOF's Waiver/Release forms and save time during registration. These forms are good for (1) year and will be kept on file at LOF. Future visits (within that year) to LOF will only require each player to 'sign in' on a daily player log.

ALL players must have a Waiver/Release form on file at LOF to play. This form may be obtained by clicking here.

ALL players must also have a Model Release form (bottom half) on file at LOF to play. This allows us to send any/all photos taken at LOF to paintball media sources and to use them on our website. All players under 16 must also have a Medical Release form (top half) on file at LOF to play. These forms may be obtained by clicking here.
